To provide civil registration services with the highest degree of accuracy and correctness guided by the implementing civil laws and regulation and to serve the populace of Tinambac with integrity, efficiency and competence regardless of their class, beliefs and culture.

To provide the basic services on civil registration in accordance with the applicable rules and regulations of the government.

REGISTRATION OF VITAL EVENTS THAT AFFECTS THE CIVIL STATUS OF AN INDIVIDUAL
  • Certificate of Live Birth

          On Time: 30 days from date of birth

          Delayed: Beyond 30 days from date of birth

  • Certificate of Death

          On Time: 30 days from date of death

          Delayed: Beyond 30 days from date of death

  • Certificate of Marriage

          On time: marriage with marriage license (15 days from date of marriage)

  • Marriage with exceptional character, ex. Arts. 34, 27, 28 & 33 (30 days from the date of marriage)

          Delayed: beyond the given period in timely registration

Certificate of Live Birth (COLB)

Schedule of availability of Service:

Monday to Friday             –           8:00 A.M. – 5:00 P.M. (without noon break)

On Time: 30 days from date of birth

Fee/s: NO FEE

 Requirement/s:

  • Four copies of accomplished Certificate of Live Birth
  • In case of an illegitimate child and should the father will acknowledge/admit the paternity of the child, he must personally appear to the MCR Office to affix his signature on the Affidavit at the back of the COLB form.
  • Current year’s CEDULA or Community Tax Certificate (CTC) of acknowledging parent.

  • In case of marriage with exceptional character, the solemnizing officer shall affix his signature on the Affidavit at the back of the COM form. This affidavit must be notarized or signed by the administering officer.

  • Current year’s CEDULA or Community Tax Certificate (CTC) of acknowledging parent/s.

Note: Child born before the New Family Code of the Philippines, both parents will acknowledge.
